Before we share you story on Our Health Space, we will make sure you understand this information and sign a consent form.
• Your story can be written with words.
• Photos of you you or other things.
• It could be a video.
• Use recordings of your voice.
• Be an artwork
We also ask if you give consent to share your story:
• On our websites
• On social media like LinkedIn or YouTube
• In podcasts
• In workshops and webinars
• On TV or in the newspaper
• In our newsletters
• In printed information.
When we share them everybody will be able to see or hear your story.
This includes people you do not know.
To say thank you for sharing your story, you will receive a $100 gift card for time and effort.
After we work together on your story, if you decide you don’t want it on the website, you will still be given the gift card.
If you give consent, the Centre collects information about you in this form.
We share your information to tell your story.
We only share your contact details with our partners
Our members are people and organisations who do Centre work.
You do not have to give us this information.
But, we cannot share your story without this information.
The University of New South Wales is a member of the Centre.
We will say the University for short.
The University keeps your contact details safe for the Centre.
Only people who work for the Centre or the University can see your contact details.
People can only see your contact details if they need to for their work.
You can get a copy of the information we keep about you.
You can ask us to change or delete any wrong information about you.

Copyright is the legal right that protects someone’s work or story from being copied, or used in other ways without their consent.
Copyright protects things a person creates, like writing, artwork, music, photographs or video.
Giving consent to share your story means:
You will still own the copyright for your story.
You are okay with the Centre, UNSW, or our partners making changes to your story.
This is is called editing. We will do this with you.
You agree that as far as you know your story does not copy or use someone else’s copyright material without their consent.
You agree that UNSW does not need anyone else’s consent to use your story on the website.
You can choose to have your name on your story.
Or you can chose not to have your name on your story.
It is up to you.
